ShelterCare'sMedical Recuperation (SMR) programofEugene, ORis seeking to hire apart-time, entry-level Advocate 1to provide support to adults with high-level chronic or acute medical needs who have been referred to our program. ShelterCare Medical Recuperation is a facility that provides safe, emergency shelter for people who are homeless and recently have been discharged from the hospital after an acute medical episode, yet still require limited care. During their 30-day, post-hospital recuperation, individuals can focus on their health needs while our community health workers and peer wellness specialists help them create sustainable connections to community resources and medical providers.

ShelterCare is considered a healthcare organization. Following state rule 333-019-1010, covered healthcare providers and healthcare staff must provide their employer: Proof of vaccination showing they are fully vaccinated; or documentation of a medical or religious exemption.

The Advocate 1 earns a competitive wage of$15.84 - $17.97depending on experience.This is a non-exempt position.

*Up to 144 hours during the first 12 months for employees working 40 hours a week. Prorated for employees working less than 40 hours a week.

A DAY IN THE LIFE AS AN ADVOCATE 1

As an Advocate 1, you will act as part of the collaborative, interdisciplinary team to compassionately serve program participants, provide support to SMR staff as they guide program participants through their services plan. You will work directly with individuals experiencing homelessness who may also have mental health and/or substance use disorders to carry out service plan and access to relevant services.

You will support the Community Support Specialist or Peer Wellness specialist with delivery of services relevant to each individuals service plan. You will assist participants in achieving goals relevant to their housing remedy and transition goal plan. You will assist in enrollment and discharge of program participants. You will be assisting participants with receiving supplies needed for daily living in the shelter. You will provide assistance to program participants in connecting them to appropriate community resources as needed. You will assist each participant with access to shelter facilities such as the Computer room, garbage and recycling, laundry and Recreation Room.

You will finalize meal preparation and deliver meals to participants 2 times daily following the predetermined dietary guidelines indicated on the prepared menu. You are tasked with transporting clients or setting up transportation to medical and other required appointments. You will maintain appropriate physical and electronic documentation for program participants. You will facilitate access to mail for all residents. Lastly, you'll provide crisis intervention as needed to all program participants.

QUALIFICATIONS

Required

  • Ability to drive agency vehicles

  • Ability to successfully pass a DHS background check

  • Compassion for the vulnerable populations we serve

  • Willingness and ability to work with individuals exhibiting mental health distress and symptoms

  • Ability to obtain an Oregon Food handlers certificate.

    Preferred

  • Prior social services experience is preferred
