Position description
Home Health and Hospice cares for 400-500 patients in the community. Staff work in an interdisciplinary team that includes nurses, social workers, chaplains, home health aides, therapists (PT, OT, and ST), therapy aides, massage therapists, music therapists, bereavement counselors, physicians, nurse practitioners and volunteers to meet the needs of our patients. The department cares for individuals with healthcare needs in their home, which includes from birth to death and all types of medical conditions. The patient population is one of the most varied populations found in healthcare!
The Home Health Aide/Hospice Aide is a trained person who provides personal care and assistance in activities of daily living for people in the place they call home. The Home Health Aide/Hospice Aide works independently under the delegation of a Registered Nurse and/or when appropriate a Physical, Occupational or Speech Therapist, and at times under their direct supervision.
Qualifications
Valid driver's license, reliable transportation, the ability to transport patients and acceptable driving record in accordance with Mayo Clinic Policy. Meets Federal and State guidelines required to work as a Home Health Aide in a certified licensed Home Health agency. Certified Nursing Assistant, on State of Wisconsin Registry. Current BLS certification for healthcare providers.
Additional qualifications
High School Graduate or GED equivalency preferred. Has worked as a Certified Nursing Assistant within the past 24 months preferred. 18 years or older preferred.
Ability to read and communicate effectively in English. Basic computer skills and experience with other technological devices (i.e. cell phone, black berries, etc.). Ability to work irregular hours (days, evenings, holidays, weekends). Self-motivated/independent. Able and willing to drive in all weather conditions and cover service area. Able and willing to work with all populations served.
License or certification
Certified Nursing Assistant, on State of Wisconsin Registry. Current BLS certification for healthcare providers.
